I was thinking that my app for pr was finished, but apparently I need my physician to sign/examine me??? i dl'ed the form but i didnt understand it...does my physician just need my vaccination record or need to conduct an exam??

Yes A physical would be wise..the school has the requirement that a GP evaluate your mental and physical health before enrolling at the school. Also he or she needs to check your vaccine record for Hep B and others (basically have it all up to date).

Just have that done and send it in to the admissions office.
