IBS in intern year?

I feel embarrassed talking about this. But for the past 1.5 years my IBS (mixed constipation and diarrhea) has gotten pretty bad. I have had a normal colonoscopy and my PCP keeps telling me its my fiber intake/poor diet.

I admit its not a perfect diet, but I have made serious improvements. Really with a $46K per year salary its not like I'm going to be able to eat organic and completely cut out carbs completely.

But I feel like its been easy to deal with my symptoms as a medical student (except for the board 9 hour exams where I starved myself the day before). But I wonder how it is with intern year? I have a difficult time going to sleep when I am constipated and won't go to sleep without a suppository in sometimes. But I wonder what would happen if I ddi that during call night?

I was wondering if anyone has any coping suggestions? I never get the right medication balance right either. I have done Amitiza, Docusate, Ranitidine, and Fiber, but my PCP and I both agree its diet related.


I suppose I could try more exercise? I've just moved to my new place of residency and I think the heat has taken a toll on me, I've never realized how humid and hot some places can be. Perhaps it's made me a little anxious is what I'm thinking. But I literally even have a difficult time swallowing things during exams. I think its sympathetic overdrive. I just hope that combined with the anxiety may not lead to any autonomic dysreflexia lol

Any suggestions would be appreciated.
