Hi, the following was sent to me by another user as an anonymous post request. Please see below.

"We have a wealthy donor interested in making a significant donation (no idea how much, anywhere from 6-8 figures) to our radiation oncology department. What would you ask for? Reflexion? Protons? Thanks for any great ideas!"

Endowed chair(s) a great idea - minimum here is usually 2 mil plus. Rest to support research/clinical trials for individual/department; can be specified by donor if they wish. Tech is for institution to buy, unless the donor has a specific desire.

That Reflexion and those protons have really revolutionized cancer outcomes and how we care for cancer. Tough choice!

Well, they have certainly revolutionized reimbursement. You gotta spend money to make money!

Also we don’t talk about negative proton trials or the need to inject five full FDG doses into a patient to treat a bone met.

Can I give you my bank details?

Ok, let's be serious:

I think it all comes down, to

a) how much money we are talking about (protons - not a stand-alone single machine solution - cost multiple times more than any advanced LINAC)
b) what you think will fit most to your practice (I understand CNS is your focus)
c) what competition there is around
d) what you think can provide a meaninfgul impact in terms of cancer care

If you don't have protons yet and want this machine for CNS, I would look at possible proton machines with innovative characteristics.
There was a proton machine that treats patients seated, maybe that's something?

IHMO - if someone wants to make a lasting impact an endowed chair is the most impactful. At our institution those can be through departments or through the cancer center.

Patient support funds also valuable and wouldn't argue with that desire. At that level of giving it is important to understand what is driving the donor - legacy, tax break, recognition. Would work with them to find a way for a win-win and to open the door to larger gifts in the future.
Would actively discourage infrastructure with a 7-15 year life span (i.e. equipment) but not if they want a building named after them ($$$$)

We had a donor help fund "extra" nurse navigators and it has been a HUGE patient care benefit. With that said, it has a big burn rate and will not be a "legacy" like a brick and mortar building...as human capital/staffing costs add up over the years and don't generate their own returns (though I think with improved patient care we get better reviews and referrals, but hard to quantify that).
