If someone doesn't match from the caribbean are there alternate career pathways?

Not a lot. From what I’ve seen, most decent non-clinical gigs (pharma, expert witness, insurance, etc.) generally require at least a year of residency; often times they even want board certified/eligible docs. Maybe you could teach college A&P or something like that? There aren’t going to be any options that are good as far as pay-to-debt goes.
 
This is why we always tell people not to go the Caribbean route. You’re better off not borrowing all that money to be jobless.

I’d also throw in the brand-new scam MD/DO schools into this category as well.

Brand new MD schools are not scams. Brand new DO school like Kansas COM is a scam. LCME is better at accreditation than COCA.

What about California Northstate?

I don’t know if there have been changes, but the threads I recall over the last year or two were suggested it was more of a scam than perhaps any new DO schools.

Cal Northstate finally passed its accreditation, and the current school leadership seems more competent. Financially, however, it's still a for-profit institution, and it will take time to overcome the legacy of their struggles.

 
N=2 but Cal Northstate applicants were red flagged when I was on the resident selection committees both where I did residency and fellowship. The handful that were interviewed were either really outstanding on paper and/or had ties to the area but were definitely scrutinized more during rank list discussions, which was reflected with where they ended up on those lists; especially compared to DO applicants.

Edit - this was also with their first couple of classes that graduated, and Idk if things have changed beings I’m not involved with that process currently.

If you can string a few Spanish words together, apply for a 1-yr non ACGME accredited internship "internado" in Puerto Rico (PR). Once you are done, you will be able to get a PR license. Then, move to Florida and apply for an Area of Critical Need (ACN) license, which will allow you to work in some designated clinics that are all over FL.
