if you want to get a cardiology fellowship right after residency, then you apply during your pgy-2 year. if you don't care, then you don't.
i have no idea if you can do em first, then do cardiology. i would submit that if it could be done, it wouldn't be smart time wise (prelim/transitional intern year, followed by 3-4 years of em residency, and then apply to cardiology). for what its worth, i believe you have to go through im in order to do cardiology these days.
onco... i'm assuming you're referring to oncology. oncology is a fellowship after im. so, to do im, then do onco (2 year fellowship), and then want to do cardiology would seem a bit excessive.
You have to complete an internal medicine residency. It can't be EM, or anything else. (Unless you want to do pediatric cardiology, in which case you must complete a peds residency.)
You have to do a medicine residency (3 years) to do cardiology fellowship (3-5 years). If you want to do pediatric cards instead of adult cards, you have to do a peds residency first (3 years).
Apply to cards fellowship during 2nd year or 3rd year. If 2nd year, you could start right after IM residency. If apply during 3rd year you'd have to work for a year or do research.
Oncology = same deal. Do internal med or pediatric residency first, then an oncology fellowship.
