IM Primary care vs wait a year

I am Ross university graduate few months ago
step1 230 step2ck 218 (second attempt) step2cs pass.

I didnt sign the contract yet but I am considering IM primary care position. Concern is that PD specifically stated that program does not support fellowship other than primary care related. No letters offered.

And I really like options opened rather than closed. This is outside the match. I did not match for 2019 because I applied late and failed step2

I need advise on what I should do? considering both ethically and possible options.

I am considering waiting a year vs. Transfer vs. Graduate then try fellowship anyway

Definitely take it. You should be kissing that PDs feet for even giving you a chance as an IMG with a Step failure.
 
You should probably take this spot. If you applied in the match with your passed CK and didn't get a spot, reapplying next year is likely to result in the same outcome.

Transferring is very difficult and honestly unlikely. It is very, very difficult to be a PGY-1 in one program, and then start as a pGY-2 in another. You won't know the people/system. Positions aren't centrally advertised anywhere, and it's unlikely your PD will help you with this. So if you take the spot, you should assume you'll finish it, don't plan on transferring. This isn't like community college.

As far as fellowships, if you're interested in ID or Nephrology, anyone can get a spot. If it's a pure primary care program you should assume you will not be getting a spot in Cards, GI, or Critical care.

Take what you’ve got and don’t be stupid. You’re an IMG w a board failure who failed to match in the NRMP match - your chances of successfully matching a second time around are essentially zero

Did you really need to talk to your mentors about such a thing? Are some med students really that uninformed? Nothing against you OP, but I am having a hard time believing that you need to even think about that?

Count your blessings! Also, do primary care track IM positions really lock one out of the 'other than Neph, ID, and Rheum' fellowships?

From what I understand it's the not having a letter from the PD that will be a problem.
