I initially was planning to be a radiologist but recently have been reconsidering my decision. However, its too late for me to apply for IM. I did apply to transition year. My question is do IM residencies accept students as PGY2. Can the transition year count as PGY1?

Depends on the transition year. Some years are labeled "transitional" year but are really the same an IM prelim. If you've done any peds, surg, or OB, those months will not count and you'll have to spend some time as a PGY1 in any program that might accept you.

You will need the blessing from the Rads program, as you are contractually obligated to show up there in July.

EDIT: Wait. Maybe I read wrong. You want to apply to IM late? Or like next year? I'm not exactly clear what's going on here.

You can most definitely find PGY2 openings in IM programs. If you're just applying now to transitional years, try to rank TY programs associated with good IM programs higher. That way you sort of have a foot in the door and sometimes a program might just let you stay on as categorical IM if they like how you performed (and they already have your application and you've already interviewed there). Sometimes, PGY2 residents drop out or transfer programs for whatever reason, and thus there may be a bunch of spots out there that really need a replacement by the time PGY2 rolls around, or during PGY2. These spots dont tend to be publicized anywhere though, so you really have to be proactive in seeking them out. The upside is that you dont need to go through ERAS or the match to get them.

If you do find a PGY2 spot in a program other than the one you did as an intern or med student, you may or may not need to redo a few months there as an intern to get the hang of the system. It does seem like a downside extneding your training by a little bit, but i personally see it as a good thing, so you dont get dropped right into the fire. But that's why i recommend trying to get a TY internship (or even better--prelim IM) associated with a decent IM program where you wouldn't mind staying for categorical IM if it came down to it.
