Does anyone know what percent (if any) of applicants are rejected up front after interviews (promptly following committee review) vs put "on hold"?

Every place is different. In general, a formal rejection immediately after interview is uncommon (unless a particular school changed its procedures and policies). If it did happen to you, you must have ticked-off someone, personality wise. If so, then, self-reflect what happened and make yourself a better interviewee for the next interview elsewhere.
