I've been watching this story, but just saw that the responsibility for the mistake was given to a pharmacy tech. Very sad situation.

Good pharm techs are so important. They can make things a lot easier for the pharmacist. The issue is that there are not strong regulations are who can be a pharmacy tech. In IL, anyone with a high school diploma can be one.

true....pharm techs do make pharmacists' lives much easier and less stressful than it would be without them......

Question: So it was misshelved. Who did the check (QC) when it was taken from the shelf and given to the babies? Weren't these bottles labeled?
