I would go ahead and release your application if its complete. What are you waiting for? You cannot change anything at this point.
A good Step 2, research, and good recs overcomes the poor step 1 score in general. Neuro is not derm or rads in terms of competition.
As always if you want to go to Hopkins, Mayo it may be a reach, but if you really have some dream programs apply and find out.
If you have decent grades, good evals/letters, research, etc you should not have too much trouble getting interviews.
