You must pass all steps of the USMLE including the CSA to:

a) obtain your ECFMG certificate (not including Step 3)

b) to obtain an unrestricted license to practice medicine in the US (this does include Step 3)

c) to obtain a DEA number in the US.

Therefore, if you do not pass the CSA and are interested in doing a US residency or working in the US, you must take it again.
