Increased technology and increased myopia questionnaire

Hi everyone, I'm currently a NECO student doing a survey - we are looking at the increased use of technology in today's society (i.e. smart phones, tablets, laptops, etc) in comparison to the increase in myopia. We are trying to get a wide variety of ages and geographic locations. Our questionnaire takes about 5 minutes to complete and would really help us with our study.
