Information card for Roseman (USN)

Hey guys,

Can anyone confirm what an information card is? It suggests that it must contain:

individual student’s DENTPIN, AADSAS ID, email, phone and other contact informatio​

n on a 3x5 index card...

so do I buy an index card from a nearby store (flash-card?) and write the information above and mail it over (along with my processing fee)... Or is there a link that I am missing to download and fill-out on the site????


On the side note, anyone know if they are accept Canadians readily... haha
