I was not put on probation or anything of the sort. On my unofficial transcript online there is no indication of academic suspension or anything similar. The professor told me to consider it a warning. Despite this, "permanent record" makes it sound like the sort of thing professional/graduate schools are meant to see.. I feel like I will definitely have to admit to it in some fashion and explain myself. Does this mean I would have to put on my AMCAS application?

If you are not certain whether or not you have been the subject of an institutional action, contact the registrar, student affairs officer, or other appropriate party at the institution for confirmation of your record.
I know I'm drudging up an old(er) thread again, but I just heard back from my University that there is currently no disciplinary background existing on me. I am clearly fortunate that for whatever reason, the professor decided not to send in the report.
