Institutional Action

Did your instittution (school) take any action beyond what your teacher did? An "institutional action" requires the institution to take an action. I guess you could report that they put a report from your teacher in your permanent record. That's an action. Whether that would derail your application is questionable and whether that even rises to the level of "institutional action" is questionable.

If you had been sanctioned for cheating (such as being placed on probation, suspended or expelled) then you would have a serious problem on your hands.

You have two choices: you can forget about med school or you can go ahead, take the courses & MCAT, report the IA and hope that adcoms have mercy.
 
Obviously, the professor didn't find it to be a serious enough offense to fail you on the paper OR the class. He probably reported it to simply have grounds for more aggressive measures if you continued to have issues with academic dishonesty. I don't think this is a one-time deal breaker. Could it possibly come up? Of course. It is what it is. If it does, own it, explain it and move on. Good luck!

The professor told me to consider it a warning. Despite this, "permanent record" makes it sound like the sort of thing professional/graduate schools are meant to see.. I feel like I will definitely have to admit to it in some fashion and explain myself. Does this mean I would have to put on my AMCAS application?

Yes, if you believe it was an institutional action. If it was a warning from teacher to pupil with no action on the part of the institution, you could argue that it was not an institutional action such as probation or suspension, things an institution can do but that a teacher can not do.

Here's the relevant quote from the AMCAS instruction manual:
If you are not certain whether or not you have been the subject of an institutional action, contact the registrar, student affairs officer, or other appropriate party at the institution for confirmation of your record.
 
I know I'm drudging up an old(er) thread again, but I just heard back from my University that there is currently no disciplinary background existing on me. I am clearly fortunate that for whatever reason, the professor decided not to send in the report.

the professor was probably just trying to scare the crap out of you so you wont do it again... props to the prof for being reasonable/lenient:claps: lol
