Hey,
I have a question and I don't want to ask my school. I don't know if this has been specifically asked on here... I don't believe so. I'll explain below -

I'm doing my Family Medicine rotation at a satellite city/school. My preceptor is awesome and really letting me do as much as possible. I love it. He has me doing SOAPs on my own. Doing my own work. Letting me round/seeing 10ish patients/day and then consulting with him on everyone. It's awesome. It's not stressful and what I've learned has only gotten better. My question is this -

I've been second assist in 2 c-sections this week (only been one week)...and it's definitely not a requirement for FM. But...it's a requirement for OB/GYN. Can I even use these surgical procedures I have done at FM for my OB/GYN cutoff? Can I even mention this? I don't want to get him in trouble or cause any issues. I'm not being pressured into this stuff at all. He asks, I say absolutely. My OB/GYN rotation requires being 2nd assist for surgical procedures...and I wanna know if I can add that on there? Or how I'd even bring that up on my rotations? I don't think we're doing anything wrong, but I'd like to know what options I have since I'm doing this.

I think so- at least the way my school does it. During our transition to 3rd year week they said that our OB/Gyn preceptors are generally aware of the requirements for the rotation since "there are some things you aren't going to be able to complete in other rotations", meaning that there were some things that had lots of rotation overlap and you just had to get them done at SOME point during the year.

I'm not sure why you're hesitant to ask your school about this though. You seem to be under the impression that what you're doing is outside the realm of what you're supposed to be doing/ that your preceptor is overstepping his bounds on what he should allow you to do. All of what you described seems pretty normal-sounding to me- why do you seem worried?
 
Depends.

Do you have a sheet/passport that states that DURING 3rd year (at any time) you need a certain number of C-sections, blah blah blah?

Or do you have a separate passport (as my medical school does) for each rotation, with its own sections to get filled out?

If it's the former, then go nuts. You're getting them, get them signed off on.

If it's the latter, it shouldn't be an issue, as you shouldn't have the passport anyways (we were always given them on each first day of our new rotation).

If you need to log C-sections at SOME point during 3rd year, then you should be able to log them now.

ALL of this being said, (depending on how many C-secs you need to be a part of) you will likely see TONS of c-sections on ob/gyn and will be second assist on pretty much all of them. The week I was on L&D days the only solace I had from writing those annoying notes every 30 minutes on 5-10 L&D patients was to go to every single C-section all week. Must've gone to > 15-18 c-sections in a matter of a week.

You're not going to get him in trouble. He's not doing anything that the hospital hasn't already certified him to do, and as long as you're ok with it, it's not a problem. The only way he can get in trouble from your school is if he threatens you, physically or verbally assaults you, refuses to teach, or if doing c-sections were against some religious belief and he bullied you into assisting him anyway. So go ahead and ask your school if you can count those c-sections for your OB requirement.....although I agree with others, you will be inundated with c-sections on your OB/gyn rotation.

And it's not really a complete paranoia, but just something I wanted to double-check before I went ahead and emailed them. He's not doing anything you mentioned. He pretty much just lets me hold more responsibility and do more than everyone else doing FM. I love it only because it's helping me reinforce all the stuff I've learned and get an idea of how it's applied in medicine. I'm not trying to be uber-gunner mode, I'm trying to just get as much of an education as possible from this.

They won't care, and actually may prefer that you've had some experience. It makes it easier for them, because they don't have to take the time to explain to you what to touch (and what not to touch) etc. No one ever gets mad that you've made their life easier. 🙂
 
Umm.... Ok. Ignoring the white elephant in the room, future preceptors will be glad, because it's one less thing they have to teach. Third year clerkships build upon themselves.

Yes we all know "sorry for being an *****" excuses being an ***** 🙄🙄. I didn't read all the replies and yes we did say similar things but what I really would want to emphasize from my post is that third year builds upon itself.

Sorry about snapping at you, just a combination of Step 2 plus multiple repetitive posts made me see red.
