Interfolio matching form

can someone please resolve my confusion about Interfolio document stuff?

So I understand that I have to type in my references (people who write my LOR) and get a matching form for each person. Then I downloaded them to upload on my Interfolio, but should I upload it as type "AADSAS Letter of Evaluation Form" ?

And for transcript, how does my school get a matching form from me? Is the best way to do this is to personally go visit the office?

You print out the LOE matching forms yourself, fill them out, and upload them to Intefolio. I titled mine, for example, "LOE Matching Form for Dr. John Doe."

For the transcript matching form, you can either drop by your registrar and give them the form, fax it to them, e-mail it to them, whatever. If you're unsure of what to do, just give them a quick call and ask! For one college, I dropped it off. For another, I ordered my transcript through the National Student Clearinghouse and had the opportunity to upload my matching form when I made the transcript request.
