I know many internship programs offer potential months abroad, but is there such a thing as an internship done internationally, that still satisfies all US PGY-1/licensing requirements?

Even if it is something like 6 months in the US, 6 months at a hospital abroad, does this option exist?

No.

Doing that much abroad will not meet the requirements of any of the ACGME requirements for a US residency.

It may also not meet the requirements for the relevant specialty board (some will take training done abroad toward the total required time to BE, some will not).

The converse, however, is true.

If you do a US transitional year or PGY-1 which includes 3/12 of medicine, 3/12 of surgery and the remaining 6/12 as you prefer, you will be eligible to apply to the Medical Council of Ireland, under its new rules, for Full Registration here.
