In medicine your intern year or internship is your first year of residency which counts towards licensing requirements and specialty board eligibility (or makes your eligible for an advance residency like radiology). Not 100% sure what an observership. Sounds like you are just following someone around so you can say you have US clinical experience.
So for every residency the first year is called internship?
