Internship supplementary materials

Some of my sites ask for a one page case or treatment summary. I'm probably over thinking this, but I want to make sure I include appropriate content. For a case summary, that's just background info and current symptomatology, right? Nothing to do with recommendations or interventions utilized? And for a treatment summary, would that just include very brief background info and primarily summarize the course of treatment? I cannot find examples of these anywhere. Thank you!

For a case summary, I would absolutely include recommendations and treatments. It should be a summary of the entire case. For treatment summary, I would approach it like you suggested, brief overview with diagnosis, treatments used and progress through treatment.
