Interview conflicts

I was talking with a close friend of mine and they recently got two interviews for the same day for a competitive surgical specialty. They've reached out to both and were able to get the interviews so they could go to one early in the morning, leave, fly to the other and make it but there are a lot of activities during the interview day so it is likely to hurt but we weren't sure of how much exactly.

Is there any common suggestions or best practices? One program is the home institution but they are interviewing more candidates than the other so the ratio of interviewees to slots is better. They aren't sure how many interviews they'll get and when the programs are few you don't really want to say no to any (especially when they enjoyed both during rotations :/)

Would trying to make both in one day ruin both completely?
