Technically SDN is not the place to be asking for homework questions and threads like this often get closed or deleted.

1) You've given a lot of extraneous information and you haven't provided the details we need to answer your question. You only state what the hygienist has done. You never documented the response of the dentist. Was he verbally/physically abusive or harassing in any way?
2) If you suspect something, report it and let the big boss deal with it as he wishes.
3) Take it to the next higher up.
