Hi all,

I am a 4th out of 6th year pharmacy student. I will be doing my hospital pharmacy rotation this summer for 4 weeks.

Does anyone know if it is required to obtain a NY State permit in order to do the rotation?

I have found this "Form 5" : http://www.op.nysed.gov/prof/pharm/pharm5.pdf

(From: http://www.op.nysed.gov/prof/pharm/pharmforms.htm)

can anyone whose completed their IPPE hospital rotation confirm/deny the requirement to get this permit? (70$)

I don't know what school you go to (you definitely don't go to mine), but you should have gotten a NY state intern permit after your third year. Your preceptor won't ask for a physical copy of your intern license during IPPE's, but you will definitely need it for APPE's so you might as well get it now. Technically, if you don't have an intern license, then the only thing you are allowed to do is shadow. You can't do any "work."

This is all correct. I have no idea why he doesn't already have one. Hopefully you're not interning anywhere, or going on any other rotations. Without that permit, you're a tech at best, not an intern, so that really limits what you're able to do on rotations. $70 sounds right from what I remember, our school had a notary come in at the end of P1 year and do all our class's forms for us.

Doesn't have one bc he's a 4th year a 6yr program. Aka a P2 equivalent. Still should have one by now. I thought in NYS you can definitely get one after p1 at a 4yr program?

thank you for the responses

i go to rutgers in nj and work at rite aid in nj, so i have the nj permit..

last year i stayed with family in NY and did my IPPE retail rotation at a walgreens in queens.. i was told that i didnt need the NY permit, so i didnt bother.. i wonder if that was really the case though
