Is a physician's stress/workload more variable depending on workplace setting or specialty?

Specialty meaning...... well, specialty

Workplace setting meaning large research hospital vs. sub-urban hospital vs. clinic etc

Basically, what factors have a larger influence on a physician's quality of worklife?

The combination of the amount of patients in numbers and the sickness of each are the most daunting issues for a physician in any specialty at every level of training or practice. Working in a huge inner city hospital or busy clinic, or working alone in a rural clinic can overwhelm anyone at times. Obviously, the doctor's own personal issues brought into the workplace, issues with the medical management around you, and insurance and bureaucratic problems are difficult contributing factors to deal with. Also, there is a time bell-curve consisting of age, education level, experience, and time in service that can lead to burnout for a physician in any specialty.
