So conventional wisdom says that the PS should revolve around a "unifying theme", much like the typical college essay. But is this realisitic? I don't have one particular reason for pursuing this field (my neighbor's breast cancer did not give me the sudden urge to look for a cure). It is a bunch of factors together that make me suitable.

Write it the way it is.

A theme will make it easier for you to form ideas and put them on paper. It's up to you.

Just about every PS in one way or another "unites" leadership, science and a desire to help others, but I wouldn't reach too far or add too much fluff to come up with some theme. Just explain why medicine is right for you.

Although it's unrealistic to talk about one thing your entire PS, you also have to make sure that it doesn't sound like a lot of random scattered examples. The "theme" is just something to tie it all together. (for example, your passion for learning, or how you want to help people....... etc). I would suggest writing down a bunch of things you want to talk about and seeing how you can make them relate.
