I was thinking about cancelling my Step 1 test appointment (via Prometric homepage) and then extending my eligibility extension period as I felt I needed more time to prepare for the exam.

Would any of this information (cancellation, eligibility period extension) be reported to residency programs along with my future Step 1 score? I am a bit worried that these wouldn't be looked so fondly upon by the program directors when I apply for residency later. Thank you for your help in advance.

No, nothing is reported until you actually start your test.
For example, you are on your way to the Prometric test center but your car breaks down and you are unable to reach on time then all you lose are some $$$ depending on your situation (rescheduling fees etc).
Contact them for details.
