Hello,
so I need to send a transcript from Berkeley Extensions but I am in school in Pomona at the moment so I cannot drive to berekeley to drop off the request form. Is the form required or can I just do the online transcript request and that will be fine? thanks for your input!

If you're referring to the AADSAS transcript matching form, then yes, it is required.

OK there are two things here:
1- you fill out a "transcript request" and submit to the school you want to obtain the transcript from.
2- you print out a "Transcript matching form" from your AADSAS application, fill it out and give it to whoever is sending your transcript so they can "attach" this form to your transcript when they send it. The Transcript matching form is required
