Is there really such a thing as a second tier program?

Help me out here, oh dermies.

Is there really such a thing as a second tier program? In other words, for a relatively competitive candidate (not one of the perfect scores/AOA etc. types, but a solid applicant with very good research/letters/etc.) with a tie to a less popular area, are they at a relative advantage? Or is the competition that fierce for EVERY program?

Do people rank places like Iowa and Case Western (which colleagues tell me both have good derm programs) lower 2/2 location?

It depends on candidates, I think, from what I hear from residents and applicants (not that I am a huge derm expert).

How's life? I haven't checked these boards in over a year until last night - I'm now at a (very) large Pharma as Associate Director of Medical Sciences in their research division, and one of my "areas" happens to be derm.

Precipitated a question or two.
