Is this an institutional action? (Not substance/cheating related)

Very early second semester of sophomore year I got depressed. My parents were worried about me so they contacted the school to see if there was anything they could do to help.

Here is where I have questions about if my experience is an institutional action (IA) or not. After my parents contacted the school, I had to meet with the dean to discuss future plans at the school. In writing they recommended I be assessed by the school's counseling service and seek regular counseling outside of the school at a private office. I agreed with being assessed and went for my assessment. I ended up taking a voluntary medical withdrawal from the entire school a little bit later as I was still very unhappy.

This was 5 years ago. My mental health history did not follow me and i'll be applying to medical school next cycle with all the classes/experiences a premed should have, and my application is "strong"

However, do I have to answer yes to the IA question on the application since I had a meeting with the dean and they recommended assessment/counseling? For reference I contacted the student affairs office and they told me I "did not have a community conduct or academic violation record" but said nothing about sanctions like the assessment/counseling recommendation.

The Dean’s actions were not disciplinary in nature. He was being helpful and showing concern for your well being. Not an IA. Good luck.
