Is this book useful or not in first 2 years?

I am nearing the end of my 2nd year. With the risk of being presumptious and offending those with greater knowledge, I would say, no. There is likely no need to expand your learning to decision-making during the basic sciences, just another book there isn't time for...
 
Since when were first and second year students required to make decisions?

Save your money up for an UpToDate subscription in third year- the best investment you can possibly make in medical school (if your school doesn't subscribe institutionally). Now THAT will help you with decision making when you need it. (And presentations, and pimping, and impressing your attending with primary source docs, and so on)
 
Never even heard of it. I prefered the USMLE first aide and Underground clinical vignettes. Totally worth the money, completely prepared me for my second year exams and all I studied for step one. Also got me set to think clinically for third year.
