1.) Osteopathic Internship
2.) PGY 2 & 3 (Maybe repeating PGY1) Allopathic Internal Medicine Residency
3.) Osteopathic fellowship?? (Cards, Nephro, etc.)

Also...if I elect to NOT do my DO internship, can I do a fellowship in one of those five states (allopathic program of course)?
 
Why would you? Just go to an allopathic program and stay the course...

I don't know if PA, WV, or MI fellowship programs would take me without my rotating internship?

Atlas;

Osteopathic fellowships require completion of an AOA-accredited residency. So, no, your scenario would not work. However, if you completed an AOA residency, you could enter an ACGME fellowship, then petition the AOA for accreditation of the post-graduate training. Or, you could enter an AOA fellowship after an AOA residency and not worry about it. Finally, you could complete an ACGME residency and fellowship, then petition for accreditation of both residency and fellowship. Too many choices, huh?
