In Italy medicine study is quite theoretical, and for the strong theoretical knowledge italian doctors are appreciated around the world. But clearly it means that there is very few practice during the 6 year of medical school, you practise more during the specialization (which is after medical school and it lasts 4-6 years).
Of course, if you want to practice more you can go abroad in Erasmus program or something like that, or you can ask directly the teachers if you can stay more in the hospital.
