anyone know good resources for iv to po conversions? Is there a chart out there? I know there's lexi and global rph, and google. But I was hoping for a comprehensive chart.

what's the conversion of digoxin iv to po? valproic acid? I know hydralazine IV to po is 1:2 and keppra is 1:1:?
