Job Dilemma: big practice vs small practice

I'm coming out of fellowhship and facing a tough decision to choose between two jobs in a location that interests me. They are both Academic.
One is a larger practice in the suburbs with 2 locations and highly equipped clinics and OR, they already have 3 active surgeons in my subspecialty. They offer a higher salary (guaranteed salary, 45K higher than the base salary of the other job, but no additional incentive for the fist 2 years).
The other one is a smaller practice with less modern clinic/OR but more prestigious institution in the heart of the city. I'd be replacing a retiring surgeon. They pay less, but there would be room to make extra cash on the incentive. I think the benefits are comparable.

Any advice on joining a bigger vs a smaller group?
Fancy with bells and whistles vs less fancy?
Guaranteed salary vs base+incentive?
Name recognition?
