Where are good resources/book for things to ask and ways to prepare for joining a practice and transitioning from residency to the private world?

If you find a decent resource, let me know what it is. This is an area that is sorely lacking in medical education. I pretty much educated myself by asking attendings/peers and poking around on the interwebz. Fact is, a lot of it is on-the-job training. It's like they say, you can't teach wisdom. I'm more than happy to entertain specific questions, if you have any.

I recently read "Waking Up Blind" by Tom Harbin, which was a riveting read (I definitely recommend). He also has a book about making the transition from residency into practice, "What Every Doctor Should Know But Was Never Taught In Medical School." I haven't read it but he it may be helpful, particularly as he is an ophthalmologist. Here's an interview with him on SDN.
