I'm having a hard time finding the answer to this question. I'm not very familiar with the Joint Commission requirements yet since I'm on my first rotation. Here's the question:
Do you have to have indication for use for each drug written into the formulary? Example: If an MD wants to use Seroquel off label for insomnia (and writes"prn insomnia"), does the P&T Committee have to add that indication to that medication in the formulary for it to be used that way?
If you are able to answer this question please give me the link to where you found the answer...I've searched and searched.
