Does anyone know of a comprehensive list of the top journal articles (landmark articles) that all internal medicine residents should read. I know the University of Colorado at Denver IM residents have a Journal Club called "Journal Club Classics" which is a year long journal club that goes over 75 landmark papers...does anyone happen to have this list? I could go through and put a list together individually but I know a lot of IM residency programs already have things like this.

I don't think there is one comprehensive list. If you want the U of Colorado's list, why don't you email their PD or something? He'd probably be flattered.

We could make our own list (on here).

I'll start. The AFFIRM trial (study of rhythm control vs. rate control strategy in atrial fibrillation, and its affect on long term survival). I believe it was in NEJM somewhere around 2002, but I'll check and find out.
 
Maybe copying and updating according to the following format would help organize the list (in no particular order, maybe alphabetical?):

CARDIOLOGY:
1. AFFIRM - rhythm control vs. rate control in atrial fibrillation
2. COURAGE - maximal medical management equivalent to PTCA in stable angina
3. JUPITER - Rosuvastatin significantly reduced hsCRP levels
4. ICTUS - Invasive vs Conservative Treatment in Unstable Coronary Syndromes.
5. ENHANCE- Rosuvastatin + eztemibe did not decrease intimal wall thickness despite lower LDL vs. rosuvastatin alone

GENERAL IM:
1. ??

HEM/ONC
1. LMWH vs Coumadin for anticoagulation in cancer patients. N Engl J Med 349:146, July 10, 2003

PULM/CRITICAL CARE:
1. NICE-SUGAR - 90 day mortality benefit with glucose < 180 vs. 81-108
2. Rivers, et al - Early goal directed therapy in sepsis
3. CORTICUS - Hydrocortisone in septic shock

NEUROLOGY:
1. SPARCL - 80 mg Atorvastatin reduced recurrent stroke and CV events with a small increase in incidence of hemorrhagic stroke. Numerous post-hoc subgroup analyses have been published as well.

ENDOCRINE:
1. ACCORD - stopped early because of an increased risk of death in patients who underwent intensive blood glucose lowering.
2. ADVANCE - showed a reduction in the progression of albuminuria with intensive glucose control, there was no effect on cardiovascular-event rates

NEPHROLOGY:
1. ???

GI:

An oldie but a goodie showing reduced sx's and a mortality benefit from the addition of Enalapril to Stage IV heart failure.



General Medicine: JNC 7 December 2003 in Hypertension. 2003;42:1206
Not really a landmark trial but important nonetheless as a guideline in BP management. Latest revision due soon.
