I had a case report that was a cardiology case go into an ER journal a few years ago (can't recall the name just now). Also, if it's an interventional case, I think some of the interventional journals might take it (or take it as an interesting image, etc.). Texas Heart Foundation I think has a journal that maybe takes interventional case reports, too.
