Just how bad is it to back out of a job offer?

Not that I would ever do this, but say I accept job offer #1, then get an offer from job #2, what would happen to me if I backed out of job #1? I assume I would get blacklisted from employer #1, but would any other employers be able to find out that I did such a thing?
 
I would assume you would more or less be blacklisted for some time.

I know a good amount of people who have done this, but because they matched with residencies
 
We're having a similar discussion right now on a non-health care website.

Unless you did a major flounce, it really shouldn't matter. Better for everyone involved to take a job that you think is the better fit for you than one that isn't. BTDTWTT. 🙁
