Has anyone had any luck cold calling critical access facilities in Kansas for locums work? Is this even still a thing or are facilities wanting to use staffing agencies or CMGs?

Still a thing. Just call the ER and ask to talk to the medical director, or just get their contact info. I landed another side gig in CA this way a couple weeks back.
Was this at a site where docs are hospital employees? Because that will make sense. If an ER is staffed by a CMG I’m not sure how cold calling would bypass the CMG and you’d work directly for the hospital to staff their ER.

Cold calling wouldn't bypass the CMG, but if that particular hospital needs help, then they should be able to let you know who to talk to at the CMG or wherever in order to get hired. Of course, if the ER is staffed by a CMG, then you'd be working for the CMG, not directly for the hospital.
At my new gig, docs do happen to be hospital employees, but it really shouldn't matter in terms of you getting hired.
