How bad is it not to have any sort of leadership throughout undergrad? I have done volunteering, clinical work, TA and tutoring, research, and a part time EMT job, but I haven't had much time to join many clubs on campus and get leadership within those clubs. Is this looked upon unfavorably?

no you're fine. just don't apply top heavy... or avoid top 20 in general
 
^ I don't know if I agree with that. I don't think a leadership role is 100% necessary for top 20s.
 
You could probably use the TA/Tutoring if you spin it right. I wouldn't worry about not being in club leadership. At my school clubs would go through 57437378 officers just so everyone could have their turn.

I'm going to go a bit against the grain and say that I don't think having a discrete, official leadership position is a must, and I don't think it will be looked upon unfavorably. However, I do think you need to be able to answer the question of how you've demonstrated leadership in the positions you worked and/or volunteered in. Personally, I never had an official "leadership" position like President or Manager, but I could point to many experiences where people looked up to me, respected my opinions, and asked me for guidance. And honestly, as far as leadership is concerned, that's what I feel is most important.

Your Experiences seem otherwise comprehensive (except that no physician shadowing was mentioned). Provided some of them have decent longevity, I doubt leadership will be missed, except at those schools aiming to train future leaders in medicine per their mission statements.
