Stanford
Yale
Iowa
Virginia Mason

Here's how I'd do it

1) Stanford
2) Virginia Mason
3) Yale
4) Iowa
5) Mayo

I'm sure Mayo has an excellent program, but I spent a month in Rochester as a med student and it is perhaps the most boring town on earth.

Take this for what you will because I didn't interview there, but Virginia Mason is pretty small and pretty popular. I would imagine that due to the small number of spots, if you really liked it then rank it #1.
