Is it too late to apply for family medicine/internal medicine residency in late November?
If so, why do the programs allow you to apply till Jan/Feb when most of the interview spots are filled up by oct/november?

To be honest, I really do not know. But if you apply now chances aren't equal when you applied September 15th. Some applicants as of the moment of writing still have ZERO invites. Unless you got Stellar scores/CV, I would save my money for next match.
 
Yes, it is in all likelihood too late.

Residency programs allow you to apply later because it costs them nothing to do so, and it's possible a stellar candidate might apply late for some reason. Unless that is you, there's not point in applying now.

Looking at 22 years of recruitment history, I can say that programs receive applications differently now than in the past. In the distant past, ERAS was a computer program we downloaded, and we received applications and supporting documents through a mailbox connection where files would be transferred directly from the ERAS upload into files within the Program Director Work Station. (I'm not a technical person, so I'm probably not describing the process correctly.). Applications would trickle in initially, and build through mid October. In 2006 (when I started keeping detailed records of the application process timeline) I received 30% of my applications between 10/2 and 10/26. Dean's letters weren't released until Nov 1st, and we did the bulk of our interviews in December and January. November 1st seemed like a reasonable cut off for receiving and reviewing applications.

Fast forward to 2018, and nearly 83% of my total number of applications had arrived the first time I opened ERAS online. Having the vast majority of applications up front, and the MSPE available Oct 1, makes the review process much more straightforward. We no longer feel as compelled to wait to offer interviews because we know we've already received most of the applications we're going to get. We've just never really thought about adjusting our application deadline to reflect the new pattern of how applicants apply. And, to be honest, we probably won't because occasionally you get that really awesome person applying late for whatever reason that you want to accommodate if you can. But I stopped telling people several years ago that Sept 15th was the opening, not the deadline.

It used to not matter so much to apply early--but now it does or you run the risk of getting lost in the shuffle.
