Late apps for ERAS - how much does it hurt?

I keep getting told by 4th years to send in my ERAS by early September. Problem is I'm in the difficult position of trying to decide between several things I'm interested in for 4th year scheduling. I'm interested in Peds Neuro or Peds v Adult PMR. I've got my IM AI in June and my Peds AI in July already assigned. For August or September I've got to try to decide what to request first, Neuro or PMR. But either way I'll won't have one of them until September meaning I likely won't get the ERAS out for one of them until probably early October. How much does it really hurt your chances sending it out in October or am I worrying over nothing?

If it helps I'm a pretty good student (top 20%) with decent clinical grades (couple of HP, a couple of H) and an excellent step 1.
 
It hurts a lot. MSPE release is 10/1; Many programs will have their interviews set to go out as soon as that's released. If you apply after that half the interview offers may be gone already. ERAS is released 9/15 this year, try to get it out on that day.

As much as this is going to suck, I'd just apply to all 3 right off the bat or however many you are still considering in September. Write 3 personal statements, see if you can get someone you trust to write a letter and change the specialty. (Maybe a Peds person for Peds and Peds Neuro, and IM for PM&R). You can get your second or third letter a bit later, but you can get your screenable criteria in on the first day.

Try to get your medicine AI switched. Beg a classmate. Surely there is someone else in your class trying to do medicine that would switch.
 
Thanks for the advice, everybody. Unfortunately, the IM AI is an early AI which is part of my 3rd year and can't be changed. But I think I'll end up scheduling my PMR elective in August. I have a better story for Neuro cuz my daughter has epilepsy so the the PS will be easier to write. Plus the Peds Neuro program director knows me so it'd probably be easier to mad-dash an LOR in September in Neuro. Maybe, I'll hear angels singing hallelujah while I'm on PMR so slapping something together last minute won't be necessary.
