Letter of Intent format??

I'm having difficulty writing this email, as I'm not sure what I should be expressing, or how I should go about it.

I want to send an email to my state school (MD), which I am currently on the waitlist for, and express my interest in it over an acceptance to a mid tier MD. Do I even mention my acceptance? If so, how to do that without sounding like a ****? Do I just summarize again the reasons why I want to attend, let them know about my current endeavors, or re-express my interest in medicine? Also, the acceptance I have is to an MD/MBA program only, not sure if it is smart to mention that I am now having my doubts about it even though I chose it and cannot turn back.
 
there is no need to mention any other school..... dear so and so, I'm writing to just check in to restate that I'd really love to join you in august. If you think that might be possible, I'd love to hear from you.
 
Actually, the only reason an LOI might even be effective is because you have another acceptance. You need to mention that you have been accepted to School X but much rather prefer School Y for the following reasons
Is there a tactful way to say this? Or just be direct? I, like the OP, feel like it's very easy to come off wrong in this scenario. I was planning on not mentioning my other options at all.
 
If you want to be tactful and not risk anything, dont send the letter
If you want to have any impact, which is unlikely in any case, then be short, respectful and direct. A paragraph or two at most; no more than 1/2 a page. You are not re-arguing your application.

To the members of the committee

I would like to the thank the admissions for their consideration of my application and for adding me to the list for alternate candidates. While I have also be accepted at School X, a fine institution where I can certainly receive a quality education. However, if I were given opportunity to attend your school, I would certainly do so over my existing acceptance. this is because school x........

If I already thanked them at the beginning of my email, how do I end it?
