I am waitlisted at a few schools and haven't written a letter of intent because I am honestly not sure if/when it is appropriate. Any advice?

write it ASAP. You want them to get your letter before they rank you on their waitlist.

It is absolutely appropriate ... and yes, the sooner the better. You need to be proactive about getting off the waitlist -- otherwise you'll languish there until all places are filled. Check out the waitlist chat on Accepted.com at http://www.accepted.com/chat/transcripts/2002/mba03112002.aspx -- a lot of it's directed toward business school, but there are some med school applicants involved, and the general strategy to get off a waitlist is very much the same.
