Letter of Rec format

I'm getting a letter of recommendation from my old boss who isn't affiliated with medicine and I was wondering if there was a specific format the writer needs to adhere to just because she isn't familiar with it... Are they generally on company or university letterhead? Addressed "To whom it may concern"? Anything I don't know of? Like I'm talking super obvious things that I might be overlooking. Thanks for all the help.

Yes the letter should be on official letterhead. Have her address it generically (ie: To whom it may concern or Dear Admissions Committee) and make sure she includes a contact number or some other way to reach her. A good length would be about 1 page.
