letter of rec request template

I started typing an email to professors, telling them med schools would like to hear about how long you have known me, what courses I took with you, my leadership, and then i thought wait a second, there must be a template somewhere (meaning a standard email that I can just copy and paste.)
so does anyone have one ready?

You should request letters in person. That said, your health committee doesn't offer you an informational sheet to provide to letter writers?

but since this is not a science teacher, he has never written a med school letter of rec so i am sure he will want to know what they are looking for. things like how long have you known the applicant, his leadership qualities, blah blah.
my school does not have a committee, even if it did i already graduated and dont think i could use them, i think they are for current students only.

My name is Billy and I am in your [Insert Class Name], I really enjoy the class and feel that my grades are indicative of my academic abilities as well as my motivation. That is why I have chosen you to write a letter of recommendation, for a nominal fee. Just think about all the fun you will have writing about my passion, and inclination towards multiple choice tests. Imagine how your life would be if you would mail merge my name in your template of letters of recommendation, as I have done here with my letter of recommendation request. Do the right thing. Oh, and I wanted to talk to you about last week's exam. Thanks!

You should still request the letter in person. Offer to send him more information if he needs it, or simply type something up to hand to him as a guideline of what should be implemented into the letter.

Chances are this teacher has still written some sort of LOR. I'm sure he will know a little something about what to write.
