Letter Problem What to do~

So one of my letter writers I was a student for and TAed for apparently did not mention I was a student in her class even though I received an A+(science letter). One of the schools I applied to (MCW) would not accept this letter because it was not mentioned I was her student. I really did not expect a problem like this to occur so when I requested the letter, I didn't specifically state that I needed to be mentioned as her previous student. Does anybody know if this is going to be a significant problem at alot of other schools?

Can't you just ask your letter writer to amend the one she previously wrote? It seems like your letter writer should be receptive if you did that much with her.
