Okay I can't find a straight answer for this...probably because its a stupid question that everyone already knows but:

For the Letter Request Forms that come up (you know, the pop up that comes up when you add another letter request during the application) - do you need to send each of these to the corresponding letter writers and ask them to include this page at the end of their letter?!? I don't really understand why these would be necessary, but I'll do it if I have to.

Yes you do! It makes filing the letter much easier for AMCAS.
 
The request form is only submitted along with the evaluation if mailed. However, your letter writer must know your AAMC # and letter #. Both are conveniently listed on the request form. It's also helpful for your writer to have this form (regardless of the method they choose to upload) as a reference of the task of writing a letter on your behalf.

Okay, so if they upload it to, say, Interfolio, is it still necessary that they include it?

No for them, but yes for you. Interfolio has an option to attach the pdf to each submission.
