Letters of Rec and Transcripts as Updates?

Is there a general consensus on how to update schools with regards to additional letters of rec or grades?

Should letters be delivered to AMCAS (how would I know schools see them?) or should I consider forwarding them to school's email addresses directly?

For grades, can I simply state my semester grades in an update letter or should I have a transcript delivered?

Updating with letters of rec is not typical. Why do you feel the need to do it?

Transcripts are only useful as updates if your grades were borderline, and you've done better this semester, but it's still pretty late in the game to send that sort of update.
